Insurance

We are Providers for the Major Insurance Companies of Blue Cross, Blue Shield, Physicians Associates, and HealthCare Partners. Due to the continuous changes in Insurance Companies and their policies of networks, Should Dr. Resnick Not be a provider of your plan; you may call your insurance plan and request that Dr. Charles Resnick become an allowed Provider and the Insurance Company will allow you to be seen in our office. Your Insurance Company will contact our office and give us permission to treat you, and our office will work out the business details with their Company.

CareCreditWe will submit claim forms to your insurance company; however we do request that at the time of visit that you pay your co-pay or deductible at the time of visit. We have contracted with many companies that are not listed, it is best that you contact your Insurance company to see if this physician participates in that plan.

If you are not part of a participating Insurance plan, payment is expected in full at the time of visit, we will bill your insurance as courtesy to you.

After your insurance is billed you will receive a statement from us for amounts that are not covered. We appreciate your prompt payment for these amounts that are not being covered. Should there be any questions about your statement, we request that you call our billing department.

Managed Care Plans require referral letters from your Primary Care Physician. Authorizations are required for diagnostic or surgical procedures. These documents are medically necessary in order for the benefit to be paid, should you choose not to go through your managed care plans required procedure, and you will be required to sign a waiver accepting responsibility for payment for uncovered services.

Payment

We accept Visa, MasterCard, Discover, American Express, Debit Cards, Cash and Checks. Returned checks are subject to a return check fee.

Patients without insurance, we require full payment for office visits at time of service, and a full payment on any surgical procedure 24 hours prior to the surgery.

Should there be an inability to pay your bill in full at the time of your visit, payment arrangements should be made at that time.

Medicare

Our doctor is a participating physician. We will submit claims to Medicare on your behalf, Medicare will pay us directly. IF you do NOT have secondary coverage, you are responsible for services not covered by Medicare, your co insurance and your deductible.

Release of Information

You must update and sign a new release of information form every year, or when your insurance has changed. Without this we cannot submit insurance claims to your carrier for you.

Prescriptions

Prescription refills are done Monday through Friday during regular business hours. Prescriptions are not refilled during the weekend.

Medical Records

Current treating patients that need medical records transferred to another physician’s office may be obtained by giving us a written letter of request to release and forward the documents to the physician.

Should you have a medical emergency you are to call 911, do not call our exchange

Please go to the Huntington Memorial Hospital where you will be properly assessed and Dr. Resnick will be notified.
